If you cannot open the questionnaire, you can download the latest Adobe Reader for free with a click on the following link:
With the latest Adobe Reader you should be able to easily open and print the questionnaire. If this still would not work with the new Adobe Reader do not hesitate to contact me. Together we certainly solve this quickly.
